Portal for Family Outreach/Family Care Meetings/End of Life Care
Visitors and Family are restricted from accessing hospitals during Covid-19. Many people rely on their caregivers for help with their care. Establishing Portal throughout healthcare institutions can faciliate increased emotional support, avenues for family and patient engagement with the care team, ensuring understanding of code status and advance care lans by medical power of attorneys, and opportunities for loved ones to speak with their loved one especially patients that may have impaired faculties, dementia, limited resources, or do not typically use a smart phone. Portal can be an avenue to families, care teams, and our ill loved ones to connect through a facility's portal. Portal hubs/booths could also be established in private rooms for goals of care discussions/end of life care decisions. Portal hubs/booths could also be established in community centers, libraries, or senior centers for communities with limited wifi/broadband/ economic resources to facilitate family conversations. A structured shared portal pod could also facilitate and assist people who may need coaching/technical help due to technological illiteracy, poor vision, or dexterity.
